Overview

The Housing Case Management Services Program takes a hands-on approach to lift seniors (individuals aged 55+) out of homelessness, within Washington County, OR.

With funding from the voter-approved Supportive Housing Services measure in Washington County, this program aims to house one of the most vulnerable populations in the country.

Our Programs

Through a three-step process, our team lifts individuals out of homelessness and into stable housing (1), identifies career skills and goals through case management (2), and refers participants to career-assistance programs (3) so they can build stable income to retain their new home. This wholistic approach tackles homelessness using proven, strategic, and individualized support.

Basic Program Qualifications

In order to qualify for assistance, participants must meet all of the following criteria:

  • Individual must be either homeless or at imminent risk of homelessness
  • Individual must be 55 years of age or older
  • Individual must be currently located/living in Washington County
