We offer intensive, person-centered case management to assist participants age 55+ in navigating the housing system, identifying appropriate housing options, and connecting with supportive services.

Our Approach

Case managers work collaboratively with each individual to develop a housing stability plan that addresses barriers and promotes long-term success.

All participants receive wraparound services, taking a holistic approach to address the full range of needs that contribute to housing instability. Our goal is to empower individuals by supporting not just their housing goals, but also their physical health, mental wellness, employment, and connection to community resources.
